Time of Collection
Because there are a number of factors that affect HPA axis function and subsequent cortisol secretion, there are several important issues to be aware of when measuring salivary cortisol in children. As mentioned above, because cortisol is secreted in a diurnal pattern, it is imperative to consider time of day in reference to sample collection and to have consistency across subjects. Timing of collection is also relevant depending on whether one intends to measure differences in diurnal pattern

Salivary cortisol is a noninvasive measure of hypothalamic–pituitary–adrenal axis function and is a useful biomarker of the stress response in child and adolescent health and temperament research [34–37]. The data of various authors on the applicability of measuring the concentration of cortisol and adrenocorticotropic hormone in samples for assessing the temperament traits of adolescents are contradictory.

This study focuses on salivary cortisol because saliva sampling is a noninvasive method for measuring cortisol, which is especially important in infants and children, while remaining reliable and less likely to confound results than other methods, including venipuncture [27]. In addition, samples can be repeatedly obtained and are easily collected in naturalistic environments without increasing ethical concerns, making this method well-suited to the study of mal-adaptive responses (see [27]).

Inconsistent findings in children may be due to variation in salivary collection and measurement protocols, sample characteristics, stress and trauma exposure, adjustment for confounders, and other factors that may influence the HPA axis (Evans et al., 2013; Keil et al., 2012). While many studies implicate dysregulation of the ANS and the HPA-axis in the pathogenesis of AD, it is unknown whether altered functioning in these biological systems is a predictor of AD risk or a state-dependent feature of anxiety.
